I don't know how to help my husband

I'm going to try to make a long story short. Over the past years my husband has had multiple jobs and he gets fed up with the company or coworkers and he just leaves... no notice or anything, just leaves in the middle of a shift and doesn't go back. Everything was going well for a while and we were able to buy our first house. Before we even made the first payment he got fired. After a few months he started a new job but then he left that one. It has been months since he worked. He had a few interviews but never heard anything back. A couple months ago he told me that he was not mentally well and needed to seek professional help. I gave him some resources to find someone but he hasn't even looked. I've thought about just making an appointment for him but I'm afraid he'll get mad and just not go, and I figured it would be better for him to find someone he's comfortable with. He's had a few times where something has triggered him and he broke his computer monitor and he threw his phone. I know he would never hurt me, but it worries me because a couple weeks ago he said that he called the suicide hotline and no one answered. Every time I ask him what caused him to get that angry he just says I don't know and completely avoids me. If I ask him if he looked for a doctor or applied to any jobs, he ignores me. I've been trying to give him his space and not push him, but I'm extremely stressed because I don't make enough money to cover the bills and I constantly worry about him, especially when I'm at work and don't know what he's doing. We just adopted a puppy because I thought that would help him but he doesn't help with training her and barely takes her outside. He will not communicate anything with me and I don't know what to do. I want to help him and I want him to be happy. I thought about leaving him a note explaining how I feel and everything since he won't talk to me, but I don't know if that will upset him. I'm sorry, this was a bit longer than intended, but if anyone can give me some advice, I'd really appreciate it. Thank you.
